In order to request a background report from Fidelifacts, you will need to fill out and sign the “Certificate of Compliance” form, and then fill out the “Client Information” form.

ORDER FORM 
This form is to be submitted along with your request setting out the searches you would like conducted. If you would like to request a search that does not appear on the Order Form, please specify that request under “Other” at the bottom of the form.
PRE-EMPLOYMENT DISCLOSURE FORM 
This form is required for ALL applicants on whom a pre-employment background report is being conducted and must be received in our office (fax or mail) before any processing begins. If a credit report will be included in your report, the Credit Release Form (below) is required in conjunction with this form. If no credit report is needed, only this form is necessary.
CREDIT RELEASE FORM 
This form is required for any individual on whom a credit report is being obtained as set out under the “Permissible Purposes” section of the FCRA, to include pre-employment applicants, prospective tenants, individuals on whom a judgment has been issued regarding monies owed (copy of judgment required), etc. There are several other permissible purposes which allow an individual’s credit report to be pulled. Please click here FCRA, which will take you to page one of the FCRA where Paragraph I “Obligations of all Users of Consumer Reports,” Section A “Users must have a Permissible Purpose,” will provide you with a list of the permissible purposes.
ADVERSE ACTION / SUMMARY OF RIGHTS 
This form is required as set out by the FCRA only to be provided to an applicant (pre-employment or prospective tenant) whom you turn down for reasons based on their credit report and/or other information contained in their background report. In other words, if a prospective tenant’s tenancy is denied, or a prospective or existing employee is denied employment, promotion or is terminated, based on information contained in our report to you, the applicant has a right to be aware of the adverse information. The Adverse Action Notice provides them with the reason that they have been denied and contact information for our company and other entities that they might contact to assist them in correcting and/or understanding the information contained in our report. If the reason an applicant is denied involves information contained in their credit report, the “Summary of Your Rights Under the FCRA” MUST be included with the Notice.
